Consecration of a Child to Our Mother of Perpetual Help

O Mary, mother of God and our Mother, your Son gave you to us as our Mother at the foot of the cross; we have recourse to you now and commend this child (mention the name) into your maternal protection.

O Mother of Perpetual Help, we take you today as his/her advocate. Do not allow him/her to desert you or do anything that will displease you. Put him/her under your powerful protection. Keep him/her always especially at the hour of his/her death. We commend him/her to your special care, we beseech thee, O good Mother, that through the love of your Son, Jesus take him/her as one of your devoted children. 

Obtain for him/her the grace to serve you worthily in thoughts, words and action. His/her thought and will power, and all that constitute his/her body, we entrust today into your hands as a sacrifice made to God his/her creator to the honour and glory of God all his/her life. 

When death draws near, help him/her, O Mary to rejoice with the angels and saints in the presence of God and your goodness. Amen!
